Holmes was living as a paying guest. Mr. Holmes had not eaten anything for the last three days and he was very sick and he also did not allow Mrs. Hudson to call in a doctor. Mr. Holmes permitted her to go to Mr. Watson, hence she had come there to inform him about Mr. Holmes.

Ans. Mr. Holmes pretended that he got illness from Sumatra. He got it when he was there doing some research work.
